That's nearly a copy for every single person in America. It's almost three copies for each person in Japan. It's nearly 1,000 copies for each person in Iceland! ]Marika has a terminal illness. "How is she?" does not seem to be a usual question that one would ask in that situation.

That and she's the one who has the lock that Raku's key would open. Mikage knows and wonders why Marika hasn't said anything knowing that she's... dying?

If not dying, then maybe some kind of disease that would take away her memories? Kinda like the situation in School Rumble?

Definitely.

That is the difficult part of reading shonen romance mangas. Progress with the different girls feels miniscule and the level of cluelessness is over 8000.

I feel like adding another girl would add another level of complexity to his existing relationships. Unless she just stalks him from the shadows and no one notices her. Lol.
